1960's Themed Surprise Party Invitation

I am in need of a design for a 45 record label that I will be attaching to actual 45 records. I need the design for side a and side b. Side A should have the following:

Name of Event: Surprise Birthday Bash
Date: Saturday, August 17, 2013
Time: 6:30pm - 12am
Featuring: Rollie D. Henderson

Side B should have the following:

Location: Knights of Columbus Hall, 1200 West 15th Street, Hutchinson, KS

These labels will be cut and affixed to actual 45 records. Let me know if there are additional details needed.
