We are the Royal Victoria Hospital Foundation in Montreal, Quebec, Canada

We need a brochure that people can fill out in order to make a donation to the Foundation.

The brochure NEEDS TO BE BILINGUAL (English and French) and include a return envelope that can be ripped off from the main brochure, and then sealed with the donation inside.

Please no pictures of buildings, only people.

I have uploaded files to show which fields must be present for donors to fill out.
