We are looking for a 2 page design for our users to edit their email preferences and submit friend referrals. You can find the fuctionality, content and text here:

* the shipping and billing address might be different so you need to show the billing if the "same as shipping" box is not checked. It can auto-hide like the page above shows if that fits your design.

* when the "unsubscribe from all email lists" box is selected, all the other checkbox options will hide as in the example page. We need you to show us what the site will look like with and without that checkbox selected.

We want to match these two pages to the general feel of these website pages:

www.motionloops.com
and
free.motionloops.com

The page should be very clean and tight. Project is for design only but must be able to be implemented in html showing button and checkbox states. Technically this must be two pages for our system - one for all the user info and one for individual email referrals (see example above).
