Somewhere in the top half of the site there should be a form for the user to fill out. The details of the form are as follows:

Title (large and bold): Contact Us or Submit Your Music
Fields to include (* = required): First Name*, Last Name*, Age, Zip Code, Email Address*, Daytime Phone, Evening Phone, Comments

Then, there will be a "submit" button to proceed to next step of form. Just below the "submit" button, should be the following verbiage: "After submitting, you will have the opportunity to upload demos of your music (optional) and provide other pertinent information about yourself, band, or group."
