Are you looking for a trifold brochure ?
Because generally the dimensions would be 11"(width)X8.5" (height) for tri-fold

But the template which you have is in reverse 11"(height)X8.5" (width).
Also there are no markings where will be the two folds in the middle.

Please suggest.

Yes, a tri-fold. The document provided is for information purposes only. It is not in a tri-fold configuration.

Please be sure to submit your designs in a tri-fold configuration. I understand that I had marked 8 1/2 x 11 when I should have marked 11 x 8 1/2. Sorry for the confusion.

Based on the designs that are coming in I think I need to add a tag line. I think that will say "Connecting Risk and Control to Achieve Strategic Objectives"

I also noticed that the address should be

15th Floor
New York, NY 10017

I must have left out the 15th Floor in my submission.
