Resumen de Diseño Web

Sleek, modern website design. I have the idea for the homepage - I will need help formatting that and also on the inner pages.

Logo centered at the top. To the left of the logo, the tabs would be "Services" and "About Us". To the right of the logo, the tabs would "For Patients" and "Referrals"
In the middle of the webpage in bold Jenna Sue font it will say "your community eye experts" with the "eye" in blue to match our logo, and the rest of the words in white.
At the bottom of the homepage should be a pseudo-transparent rectangle box. At the left side of it should be the heading "Hours" and under it Mon 10-6:30, T, W, Th 8-4, Fri 8-12:30. In the center is the header "Location" and under it will be 600 W. Putnam Ave, Lower Level (behind Citarella's), Greenwich, CT 06830. At the right side should be the header "Contact Us" with our phone number 203.900.7911 and then info@coastaleyesurgeons.com and then in italics "Walk-ins are welcome!"
In the center at the bottom of the transparent box it should say "Accepting CT and NY insurance plans"
The inner pages will include the "About Us" tab which will have "Our Practice", "Our Physicians" and "Our Staff" as the drop-downs. When you click on About Us though, I want all of that to pull as one scroll-able page. Clicking on the relevant drop-down item should take you to that portion of the page. I will provide the content for each of these.
The inner page of Services will include "Laser Cataract Surgery", "Cosmetic Eyelid Surgery", "Floater Removal", "Eye Allergy Testing", "Glaucoma", "Dry Eyes", "Macular Degeneration", "Diabetic Eye Disease", "Retinal Diseases". "Corneal Diseases", "NEW MicroPulse Laser". I will provide the content for each of these.
Under the "For Patients" tab, the drop down should include "Portal", "Insurance Information", "What to Expect", "Eye Health News". This will not be one scrollable page. These will be individual pages. I will provide the content. The Eye Health News will hyperlink to https://www.aao.org/eye-health/news-list.
Under the "Referrals" tab, I would like it to go to an innerpage where it will be like this....
1. Fax us at 1-877-991-2624
2. Download the referral form here (here should be a hyperlink to download the referral form)
3. Submit the referral online right here!
And this should have a submission entry field for "Patient Name", "Patient Date of Birth", "Patient Phone Number", "Reason for Referral", "Referring Doctor's Name and Practice", "When would you like the patient to be seen?"

That should do it! Let's work together to make a beautiful seemless page! Any considering for Search Engine Optimization while coding this website would be greatly appreciated. I would also need to keep you on-hand for constant updates to the website over time, so we can work out a deal for that!

• The services pages should have all the services on one scrollable page (so patients can learn about other services after they click on the initial one they're interested in). But I don't want it to look overwhelming, so there should be a nice smooth elegant flow from one service paragraph to the next.
