We need flyer to promote our new FMLA / Disability Form Completion Service.
HealthMark is a Health Data Services company supporting Hospitals and Medical Practices in the secure electronic exchange, delivery and integration of protected health information (PHI). HealthMark Services include Release of Information, Audit and Chart Review, Disability and Family and Medical Leave Act (FMLA) form processing, and revenue cycle support services.

I have attached some of the other collateral we have for our other service lines. We would like for the colour/imagery to somewhat coordinate with these.

The flyer we are hoping to receive is specific to the Disability and FMLA Form Processing (in Bold above). I’ve attached a copy of our workflow for your review. The value delivered to the client as a result of engaging this service is as follows:

-Reduce internal labor costs
-Improve quality
-Accelerate turnaround time
-Improve patient satisfaction
-Reduce internal phone traffic

HealthMark manages all aspects of the FMLA form completion process including:

-Review for HIPAA compliance
-Completion of forms
