I am needing a Event Ticket design for the Okmlugee High School Athletics department (https://ohs.okmulgeeps.com/). This will be a general admission ticket for ALL SPORTS. It will not be sport specific. The ticket will be 5.5" X 2" with serial numbering and perforation. It will also be a front and back ticket with the Okmulgee Athletics ticket design on one side and a Farmers Insurance Sponsorship ad design on the back. I have attached some ticket examples. The right side of the perforation must include space for the customer to fill out name, address, phone and email. It must include the ticket serial number on both portions, on the front side, of the ticket.

Debes tener

• 5.5" X 2" ticket must have serial numbering and perforation. It must contain General Admission Ticket on the Okmulgee HS Side and a Farmers Ad (Neil Hays Agency) on the back side of the ticket.
• The Farmers side of the Ticket will look similar to the business card (attached file) but please REMOVE the Direct phone (918-308-9107) and the email address (whays@farmersagent.com). In place of the email address please add the agency's web address: www.farmersagent.com/whays
• TAG LINE (at the bottom of the Farmers AD): Take the NEIL HAYS CHALLENGE: If I can't save you money on your Auto Insurance; I'll buy you Dinner!
