I would have a couple of questions, if you don't mind:
1. Do you need only a cover page, or also a table of content and a page with header and footer ?
2. I opened the file you provided and I just want to make sure the size is Letter (US size) and not A4 (European size)
3. In case you need a sample page as well, would you like to keep current header/footer or would you like it redesigned ?

Hi, yes, we are looking for a cover page only (not cover letter), but it would be nice to see a matching second page with a header and footer.

Hi, just to re-emphasize, we are looking for a proposal cover page and not a letter. (Samples of a header and footer for the inside of the proposal would also be nice). Thank you!
