Here are the changes we would like:

1. Under Office Services please put a space between "and Blood Sugar Disorders"
2. Under Office Services please use an "&" sign between Pacemaker & Defibrillator"

3. I have uploaded a picture of our PET CT and would like it placed on the inside lower right pocket of the folder instead of the logo.
