I need a form to fill out for my truck drivers to get signed on delivery. It is for a trucking company and this form will be for each machine equipment move. I have included my old sheet which I need Updated. Its going on a 9.5 inch by 8.5 inch sheet of paper. It should have room for the day, date, customer name, ordered by, P.O. Number, Project Number, Driver, Truck/Trailer, Tolls (needs more room then my old sheet need a few lines under each other for each toll during the machine move), Permits, some room for other information, Loads, Hours, and 2 lines for a Signature and Print Line.
Actualizaciones
Need to also include a Pickup Address and a Deliver Address with lines to write in. Also I need to add a Print Line (and labeled Print) for whoever is signing the ticket.
