it is a colloquialism - it means that things would be very different if there was simply an abundance of - in this case - doctors . But there isn’t so can we find an image or draw a scene that illustrates that?

I would like a concept drawing of a fruit tree with a lot of different types of doctors growing on the tree which can be picked off like fruit. - and perhaps some patients picking the doctors off and being seen by the different doctors.
