Resumen de Diseño de Flyer

I am looking for help designing a one page, 2 sided, A4 colour brochure to provide to other referring doctors such as General Practitioners that nicely summarises the various services I offer as a specialist Upper Gastrointestinal Surgeon and how they can refer patients to me. I would like the brochure to be easy to read, visually appealing, clean, fresh, modern, using blue, black and grey hues with the occasional splash of green (Pantone 376C green to be consistent with my other brand OClinic which specialises in weight management). The draft content for the brochure will be supplied. It will also need to incorporate on the front my professional name/title in an appropriate font, my photo x 2 one on each side, and nice visual icons for each of the 8 services offered, as well as a photo of my reception area, stylised simple map of where we are located, and our contact details.
