Resumen de Diseño de Tarjeta de Presentación

I need a new business card for side business. Very simple and to the point. I am a licensed massage therapist and want the colors to be warm. I don't have a logo or company name. I just need to convey that I am a Licensed Massage Therapist and have my contact information so people can contact me. I'll attach some files that I like. I am a male so I don't want it to be feminine.

One file is my existing card. I like Erin's card and the layout so would prefer a similar layout. I would like to incorporate my swimmer/biker/runner image. Doesn't have to be the exact likeness because I don't have that file but something similar to that. I would like to also include my areas of specialty of: Swedish, Deep Tissue, Sports, and Hot Stone

I would also like to incorporate a QR code on the back of the card centered.
