1. Yes
2. Please use .PSD format, or less preferably .PNG
3. Yes, 1 like/dont like design and 1 'more info'. Please use a Iphone 4 kind of screensize.

No not yet, so a logo would be bonus;). Otherwise use a font for the company name that fits well with your design. We prefer to make the logo after we know about the most awesome colours, fonts and general look & feel.
