Resumen de Diseño Web

I need a simple design for a simple idea. A one page website for now. No backend. Just looking for a design that translates the power of the idea.
The product I offer is a video made in any geo of the world as requested by the user. The user comes to the site and they see a large map covering most of the page (Google Maps API). The user submits a video request to be filmed in a specific location around the world. Ideally they will do this through the map.

Below the map is a submission form with the following fields which the user completes:

Name:
Email:
Phone:
ZIP of Request:
Request Idea:
$$:

Submit Button - goes to a Google spreadsheet.

These submissions will be turned into the specific video the customer is looking for.

Above the map will be a link that says 'Do you shoot video?' - this will click to another spreadsheet, which aggregates a community of videographers around the globe

We want something very simple and direct. The map is the focal point. The site should somehow imply that something previously not possible now is. Dream big.

Please leave space for a name/logo and feel free to pick the font/colors you feel works best
