We need a card that can be handed out to prospective clients as well as providers who can refer clients to us. Our organization is Whittier Rio Hondo AIDS Project and this program is called The Care Bridge Program. This program works to get HIV positive persons who are not currently in medical care back into HIV medical care. The program is run by a Case Manager and they help the client overcome any barriers that might be keeping them from receiving their HIV care and can give referrals for services. Examples of things they can help with are: Mental health services, substance abuse treatment/needle exchange, transportation issues, housing issues, and any other issues the client is facing. There is also a financial incentive available for anyone who qualifies for the program: up to $125. To qualify for the program the client must be a Los Angeles County Resident.
